What Is Gastrointestinal Bleeding?

In general, bleeding occurs mostly either from the upper gastrointestinal tract or lower gastrointestinal tract. The upper gastrointestinal tract includes the oesophagus, stomach and duodenum. The lower gastrointestinal tract includes the colon, rectum and anus. Bleeding from the small intestines is uncommon. Bleeding from the upper tract may manifest as vomiting of blood or passing of black, sticky stools. Bleeding from the lower tract manifests as bright red blood in the stools. Other symptoms may include giddiness, feeling faint or abdominal pain.

What Are the Causes of GI Bleeding?

Possible causes of upper and lower gastrointestinal bleeding are:

  • Stomach or duodenal ulcers
  • Oesophageal inflammation and ulcers
  • Varices ( swollen veins ) in the oesophagus or stomach
  • Tumours of the gastrointestinal tract eg stomach or colon
  • Colonic diverticulum bleed
  • Vascular malformations
  • Inflammatory bowel disease

Dr Kenneth Koo is an experienced gastroenterologist with more than 20 years’ experience and is well trained in managing all types of digestive and liver conditions.

Prior to joining private practise, Dr Kenneth Koo was Director of Gastroenterology and Senior consultant at Ng Teng Fong General Hospital. An integral member of the team which transitioned from Alexander Hospital to Ng Teng Fong General Hospital in 2015, he was responsible for overseeing all aspects of the gastroenterology division including the growth, development and governance.

Dedication to his patients and clinical practise can be seen from several service quality awards obtained over the years.

How are GI bleeds managed ?

Acute bleeds which are more severe eg resulting in low blood counts or low blood pressures will require admission to hospital for management

Initial management involves resuscitation with intravenous fluids and sometimes blood transfusions. In upper GI bleeds, other measures such as high dose anti-acid medications may also be given

Once stabilised, patients will require endoscopy to look for the source of bleeding. Once the source is located, bleeding can be treated by various endoscopic methods such as injections, clips, ablation etc. Some patients may require further radiological ( embolisation ) treatment or even surgery if endoscopy does not succeed in stopping the bleed.
